Step-by-Step Guide to Creating a Health Screening Form
In this guide, we will walk through the creation of a Health Screening Form using the Five development environment.
Step 1: Create a New Application
1. Go to the Five dashboard and select Applications.
2. Click the yellow Plus icon to create a new application.
3. Name your application (e.g., Health Screening Form) and click the Tick icon to save.
Step 2: Set Up Database Tables
1. Click on Data > Table Wizard to create tables for the application.
2. Create tables such as PatientInfo, ScreeningResults, and Symptoms.
3. Define fields in PatientInfo (e.g., Name, Age, Gender, Contact Information).
4. Define fields in ScreeningResults (e.g., Date, Screening Type, Result).
5. Create a junction table, if necessary, to connect patients with their screening results.
Step 3: Build the Health Screening Form
1. Navigate to Visual > Form Wizard to create forms for user interface.
2. For the main data source, select PatientInfo to create the patient form.
3. Add fields like Name, Age, and Contact Information to the form, ensuring proper validation types.
4. Use the same process to create forms for ScreeningResults and Symptoms.
Step 4: Validate Input Fields
1. Go to Setup > Display Types to define acceptable input types (e.g., numeric for Age).
2. Set up custom validation messages for invalid inputs to guide users effectively.
Step 5: Deploy and Test the Application
1. Click the Deploy to Development button to run your application.
2. Test the forms by entering sample health screening data and verify that everything works as intended.
